Asthma And A Management Plan For Physical Activity At School

Nearly one in every 15 students today has asthma, which is up significantly from 20 years ago. However, with good treatment and recognition of their triggers, students with asthma are still able to be physically active at school.

Their status can be evaluated using a peak flow meter. The goal is to keep the students included in the activities even if they are not able to perform the physical activities. Students can be the time keeper, score keeper or equipment manager until their health improvements.

Students with asthma are more successful remaining physically active when an asthma action plan is used. The management plan should include the students medical history, their individual symptoms, how to contact the parents and health care providers, normal peak flow numbers, triggers, medications and signed by the parent / guardian and student. The plan should have the student involvement during the development, because with the student's participation they will be more likely to follow the plan.

The final piece of the management plan should be complete access to rescue medicines. Students should be able to access their medications, administer them and determine when they need them. Teachers and coaches should not only know the student's triggers but also the symptoms that require immediate action such as coughing, wheezing, difficulty breathing, chest tightness or low peak flow readings.

When the symptoms of asthma appear during or following physical activity at school the student should stop the activity, use their inhaler and be taken for emergency help if they do not improve. Remaining physically active at school is an important piece to improved physical fitness for students to continue their fitness growth.

Asthma’s Down the Pan

A staggering 5.4 million people suffer from asthma in the UK. Asthma is an illness that sees to belong to modern, Western society and itsvalence has been on the increase since the 1970's.

They are not the root cause of someone having an asthmatic condition.

There is though a possible culprit with a degree of suspicion hanging over it; one that has previously been indirectly implicated but one that has never been closely scrutinised. As part of the interrogation I would like to look at a number of key questions.

What is the reason for someone suffering from asthma?

A relatively recent argument as to the cause of asthma observes our increased use of cleaning materials and argues that our increasingly sterile homes mean that we can not develop the immunity that we previously used to acquire through exposure to bacteria.

However, I would argue that the problem is much more direct than that.

In a nutshell, asthma arises because we put bleach (Sodium Hypochlorite) down the toilet; we leave it to do its work, sometimes overnight, we then, too often, use the toilet without flushing it beforehand.

The mixture of urine (containing ammonia) and bleach reacts to give off a small dose of Chloramines which we breathe in and which causes the damage to our lungs.

Repeated regularly the damage becomes significant.

Our damaged / weakened lungs become less hardy and more sensitive to our environment. Triggers that we would not previously have reacted to suddenly begin to affect us more. For some, exposure to these triggers leads to an asthma attack, the affected condition of their lungs making them more susceptible to attack from elements within the air that they breathe.

There remains a question mark as to whether the lungs are actually damaged or whether they are purely develop a heightened defensive system – in other words, they react more quickly to a perceived threat. This could be a significant finding, as it would determine the remedial route for those people who already suffer from asthma. If it is the former then the lungs need to undergo physical repair; if it is the latter then retraining or rehabilitation may suffice.

Why do some people suffer more than others? Why do some people suffer whereas others do not?

There are two basic reasons why there are different levels of suffering;

Firstly, people have different levels of exposure. Obviously, the greater the level of exposure then the higher the risk. Households differing quantities of cleaning materials; different strengths of cleaning materials, different ways of using those cleaning products. Our levels of exposure variies.

It is also worth noting that, not only do we use more household cleaners, including bleach, but also our attitude to the physical task of cleaning has changed. We spend less time cleaning, believing instead that we can clean as effectively by throwing chemicals at the problem. We will, for instant, put the bleach down the toilet to clean it rather than actually cleaning it manually.

Secondly, we have different levels of sensitivity. The variation in the amount and the depth to which people suffer from asthma is due to our different body mechanisms. The lung damage will heighten an individual's level of asthma risk but they must have a pre-disposition to that sensitivity.

Pain, hunger, the cold, tiredness we all feel them differently. What might be a cold room for one person might be perfectly fine for somebody else; a night of sleeplessness might leave one person exhausted whereas somebody else may disagree without feeling any different. Our lungs function similarly. The size, effectiveness and sensitivity of our lungs vary. Any lung damage effectively changes the height of the bar. It reduces their performance and, potentially, may well reduce the amount of exposure we need to a substance in order to trigger an asthma reaction.

We should also consider that an individual's diet may affect the amount of ammonia in their urine and there before make them more at risk from asthma? We know that asparagus has a high ammonia content. Similarly, dehydration and bacterial infection (particularly in women) can increase ammonia levels.

What causes the lung damage?

Our liver converts ammonia in our bodies into urea which is then expelled in our urine. This ammonia in our urine when mixed with Sodium Hypochlorite (bleach) produces Chloramines which when inhaled causes the damage to our lungs.

Of course, the dose we will be exposed to in our daily ablutions is minuscule yet it will still be damaging to us. Whatever we do, if we do it often enough, will have an impact.

It's like using a sun bed: too much, too often and you risk increasing your chances of developing skin cancer. Repeated exposure to Chloramines damages the functioning of our lungs.

Why is asthma so common in children?

Children's lungs are much smaller and more sensitive as they are still developing. They are therefore more vulnerable to any damage.

We must also consider their level of exposure to the Chloramines.

Many households leave the bleach to soak in the toilet overnight. In the morning, the first person up is six-year-old Tommy who dutifully goes to the toilet and thereby receives a shot of Chloramine.

Also, boys being smaller and standing directly over the toilet bowl receive a much higher Chloramine dose and therefore inhale more. This supports the evidence that in children more boys suffer from asthma than girls.

The damage is cumulative so, with age, lung damage becomes more common. As boys grow their nasal procurement to the Chloramine gas is reduced whereas for females, sitting on the toilet, they remain much closer. This may explain why, with age, more females develop asthma than males. It may also be that relevance, generalizing and stereotyping slightly, as women tend to do most of the toilet cleaning their potential levels of exposure are greater through doing this.

Are there any other sources of Chloramine exposure that cause lung damage?

A similar chlorine based cleaning material is used in swimming pools. Again, the chlorine when combined with urine and sweat gives off a Chloramine gas which people then inhale. In a swimming pool any urine is substantially diluted. However, it is still there and repeated exposure can contribute to lung damage. Here is the reason why so many swimmers suffer from asthma.

How should we be seeking to cure asthma?

We firstly need to eliminate the cause of the problem by ensuring we do not unwittingly produce Chloramines and expose ourselves to them. This will prevent lung damage and make us less vulnerable to asthma.

In terms of corrective treatment and whether or not we can cure those people who already suffer from asthma we need treatments that either seek to strengthen the lungs or treatments that change the functioning of the lungs so that they are less responsive to asthma triggers. The defense mechanism of the lungs is set up to watch for a particular attack but if the trigger invasion comes through a different route it can remain unobserved. The lungs would then not go into defensive closure.

This Chloramine-intervention theory as to the cause of asthma requires further further investigation. Although there is a clear finger of suspicion it is not yet a water-tight case. We should be doing everything we can to bring this case to a close.

The occurrence of these symptoms does not necessarily indicate the presence of asthma. To ascertain it, appropriate lung tests should be taken. In some cases, the symptoms are moderate and tend to disappear with medical treatment. This disease generally shows itself during the childhood and may remain with the person for his own lifetime. A person suffering from asthma experiences it in the form of asthmatic attacks. These attacks may be fatal if proper treatment is not received immediately. However this disease is not encountered until it is triggered by some external agent. Asthma – Do You Have A Plan?

Asthma affects a great multitude of people around the world. Combining the expense of missed work and the resulting loss of productivity with missed school days the total reaches the billions of dollars each year in America, alone.

Asthma is categorized as a reversible airway disease, meaning that symptoms generally improve after treatment and that the compromised airway can be restored to functional status. Therefore, managing symptoms is the key to success and reduced exacerbation or “attacks”. Causing infection, excess mucus production, and spasm in the walls of the airway, asthma can have a variety of symptoms, each of which can, and usually will, be different from person to person. Let's review a few of these symptoms and their causes.

Inflammation – Inflammation causes a reduction in airway diameter. This causes difficulty in breathing and increases the turbulence in the airflow through airway, which, ironically, causes more inflammation. An interesting fact that helps you put things into perspective on the difficulties you are experiencing when this occurs is this: If you were to take an airway that is one inch in diameter, give it a healthy dose of inflammation until it becomes half its size, or one half inch in diameter, you have, obviously, just increased your difficulty in breathing significantly. It would make sense, looking at the numbers, to say that your difficulty would now be 50% greater than before, but this is not the case. In reality, it has just become 16 times more difficult for you to move air through that airway. Talk about a change!

Mucus Production – The walls of our airways are lined with cells specifically designed to excrete mucus. This mucus is present for our own good, and under normal circumstances would be considered an ally. It helps trap foreign objects such as dust or foreign particles when it senses their presence and hold them while the cilia (little hairs that line our airways) move that glob of particle trapped mucus up toward our larger airways to be taken out. A perfect system … without the mucus is released as a consequence of triggers you are exposed to as a cause of your asthma, lung irritation, or other lung disease. If you take into consideration the inflammation that has most likely already taken place in the airway, compound it with this mucus excretion, you have just lowered your airway diameter even more, and now you have “stuff” to cough up.

Spasms – Now let's talk about spasms. Airway spasms are uncontrollable quivering of our airways, which cause you to cough. Now, this “quivering” does not have to be very large at all in order for you to feel it greatly. Sometimes they are very, very small but still carry with them a strong consequence. The coughing that the quivering causes increases air turbulence. This air turbulence, also mentioned above, increases pollution in the airway, which increases emissions and mucus production, which causes additional irritation, which can cause more spasms, which increases air turbulence, which starts the entire cycle over again. You can see why symptom management is so important.

There are many things done for symptom management. Avoiding your triggers is the first thing you should do. If you are not familiar with or have not been able to “nail down” your symptoms, keep trying to do so. It is a continuous work in progress, as triggers can change depending upon the time of year, air quality and environment (both indoor and outdoor), and your ability to avoid them. Next, take your medications, as prescribed. Do not skip doses, or stop taking them if you begin to feel better. Many medications are used to maintain your open airway, and you will only notice how much they help when you quit taking them, which, obviously, can cause problems. Keep your rescue breath with you at all times. Keep it in your purse, your backpack, your glove box, your brief case – whatever is most convenient for you – just keep it. You never know when you may be exposed to a new or worsening trigger. Medications can achieve a common goal, but use different doors to do so. We can liken it to your house – you have a back door, a front door, and probably a garage or side door. You can get “into the house” by using any door, but each door uses a different route to take you there. Meditations are the same way. In this case, they all work to “open” your airways, but there are several different paths they can take in doing so, such as calming the spasms, decreasing mucus production, or decreasing inflation. All three maintain the integrity of the airway, all three do so in a different way.

If you have had asthma for a number of years or experience strong symptoms (and really, even if you have not and even if you do not), you should be extremely familiar with your medications and triggers, and should have in place an asthma action plan. An action plan is exactly that – your plan of action in case of worsening conditions. This plan should be done with the assistance of your healthcare provider, whether it be a respiratory therapist, a trained nurse, or your physician – all of which should be asthma certified. Included in that plan should be all of your medications, when and how to use them, including how often, and when to call your physician or seek medical attention. It should be outlined specifically and leave very, very little room for question as to what you should do and how you should proceed in the various levels of severity (green zone, yellow zone, red zone) as your exacerbation progresses. The phrase “very little” can be used because in dealing with each individual situation will be something different, and unique, like a fingerprint. No symptoms or attack are exactly alike. There is always enough room for a little extra “surprise” issue to creep into even the best thought out and well executed plans.

Last, but not least, be familiar with your action plan. Hang it in a place where you can get to it easily. Keep an extra copy in your wallet. Make loved ones aware of what your plan is, where it is, where your medications are, and what you need in case you need help. Asthma prevention is symptom prevention. Be prepared. Know your triggers. Know your plan. Have your medications. And, of course, breath, and smile. You got this!
